Received from the Metropolitan Board of Education (Nashville & Davidson County) on 8/1/01 This document is printed in duplicate.
The first copy is printed on pink paper for the local education agency. The second copy is printed on green paper for the State Department of Education.

This form doesn't mention the law doesn't require your reporting to the local education agency when you discontinue homeschooling (the school you transfer to to should write the district for the child's school records) or that the law doesn't require reporting changes of address.
